American Acryl Plant Explosion; Toluene May Be Cause

A tank exploded at an American Acryl chemical plant in Pasadena, Texas, on December 9, 2009, that is reported by media outlets to have contaned toluene, a petroleum byproduct. American Acryl is a joint venture between Arkema Inc. and NA Industries, Inc., a subsidiary of Nippon Shokubai, Ltd. Here is their statement.

Exposure to toluene can cause breathing problems, skin irritation and nausea. Aromatics Online, published by the Aromatics Producers Association, provides useful information on how toluene is used. NIOSH’s description of toluene and protection requirements are here.

Proforma Safety staff are trained in the hazards related to and proper handling of toluene. Proforma recently completed a project that included fire prevention, HAZMAT response and HSSE oversight for a well intervention project involving the use of Toluene to remove asphaltine.

Fatality at Valero’s Texas City Refinery

A worker was killed by “blunt force trauma” to the head at the Valero refinery in Texas City, Texas, on December 4, 2009, as he and two other workers allegedly were attempting to restart a boiler. The two other workers were injured in the blast. Valero has attributed the accident to a “failure” of the boiler. OSHA is investigating; the Chemical Safety Board has announced it will not open a formal investigation. Current details are available here.

Never Turn Your Back On the Job

Of particular interest to us — in addition to the safety risks inherent in the procedure that went awry — is that the JSAs were not written in the two primary languages spoken at the well site. An important reminder for us to make sure everyone understands the hazards of the job.

Accident Avoided Due to Diligent Helicopter Landing Officer

An helideck safety net that wasn’t adequately secured could have resulted in a helicopter rollover and fatalities; fortunately the net released from the wheel in time.
As the helicopter maneuvered before lifting off, it slid to the side, causing the net to buckle up and catch on the wheel. The Helicopter Landing Officer (HLO) immediately radioed the pilot. Lesson to be learned: make sure all net is secured tightly. As in this instance, make sure also that all personnel are alert and watchful for dangerous situations that may arise. And an interesting point made in the report: as a rule of thumb, you should not be able to raise any part of the net by more than approximately 250 mm (7.8 in) above the helideck surface when applying a vigorous vertical pull by hand. No Frogs or Turtles Under Your Christmas Tree

Scratch that African Water Frog off your Christmas list. Turtles too. Amphibians commonly found in family fish tanks have been blamed for the strain of Salmonella Typhimurium that’s been reported recently in 25 states. Centers for Disease Control has the poop on this outbreak.

We always thought the Grinch looked vaguely…amphibian.
